The most likely cause is that the GIS layer has an incorrect number of attributes or incorrect attribute definitions (for example, a number attribute has been defined as a text attribute). Suggestions
Check the GIS attributes match those of the empty (template) .mif file. Check file can be opened in text editor, this should be text (similar to .csv). Re-export the GIS layer.
